COVID-19 Property Access Notice

For those events that require in-person services to take place at the property, this COVID-19 Property Access Notice (Form COVID-PAN) can be provided to the property owner or occupant and anyone else who intends to physically be present at the property. Using this form, all potential visitors will be made aware of the risks that come along with performing in-person activities and the Owner’s requirements for getting access to the property.
